Healthy Holston

Take charge of your health and God's Holy Temple.
Join us during Annual Conference June 11-13, 2012.

STEP ONE - Schedule an Appointment online by May 27th to participate in the Healthy Holston effort.

At this appointment during Annual Conference, you will have a biometric screening, complete a Health Risk Assessment (HRA), and meet with a health coach. (You may also schedule an appointment by calling Ken Luton at 865.293.4136.)

STEP TWO - Come to the second floor lobby of the Terrace Hotel at your appointed time to complete the process.

Remember to fast for 8 hours prior to your biometric screening or, have your physician complete the biometric screening form in advance of your appointment. Bring the completed form to your appointment.
Bring your Blue Cross Blue Shield Card with you along with a photo ID.

STEP THREE - Meet with a health coach to interpret your results and give you positive steps that will improve your health.

Participants who complete all assessments/screenings will be entered into daily drawings for great prizes.

STEP FOUR - Implement the knowledge you gain as a part of this assessment to make Holston Healthy.
